The bitch must meet the following requirements to qualify for breeding:

1) The bitch must be at least two years of age and not more than 6 years of age.

2) The bitch must be of sound, stable temperament

3) The bitch must be in good health as evidenced by a recent veterinary Examination, (Dated no later than 30 days previous on your Veterinarians’ stationary) that must include the following:

…A current Brucellosis test.

…A visual or veterinary confirmation that bitch is in season at the time she is presented for breeding.

…A list of current vaccinations.

…Medical information and veterinarian's name and phone number In case of an emergency.

4) The bitch should be groomed, bathed, and Flea treated before she is presented to us to be bred.

5) A copy of her pedigree, AKC or Foreign Registration Certificate (if applicable), and Photographs showing front, rear, and side structure should be provided.

TERMS AND STUD SERVICE POLICY

1. The stud fee for _______________ is $______________. Fee is payable upon or before the bitches arrival at Umpqua Valley Kennels LLC property.

The owner of the dam identified above agrees that the stud fee must be paid to the stud dog owner before the bitch is left in the stud dog owner’s care. (The owner of the bitch must Initial ______________)

STUD OWNER AGREES TO:

1. Take normal adequate care of the bitch while in our charge

2. Have the stud dog in good physical condition

3. Sign litter registration (if applicable) promptly and return to bitch owner.

4. Provide a return stud service of the same dog to the same bitch if no pregnancy results from the mating, at no charge. To receive the return service, the bitch owner must notify the stud dog owner of failure to conceive, in writing, not more than 60 days following the first documented mating. Date Identified pictures showing front, rear, and side structure should be providedof Bitch and must accompany notification. If the stud is no longer available for whatever reason, for the return service, the stud owner or agent reserves the right to offer a different stud to the bitch owner.

5. The Stud Owner is not required to sign the AKC Litter Registration papers (if applicable) until such time as the Stud Fee and all reimbursable expenses due the Stud Owner are paid in full by the Bitch Owner.

6. We guarantee an actual tie mating will occur or an artificial insemination of fresh semen will be provided at no extra charge.

7. Perform on premise cytology estrus cycle slide evaluations to assist in conception.

All transportation charges and veterinary expenses will be the responsibility of the owner of the bitch. There is no charge for boarding the bitch for the first seven days, after which the charge will be $7.00 per day. Any boarding fee incurred will be paid before breeding bitch is released from Umpqua Valley Kennels LLC (The owner of the bitch must Initial ______________)

BITCH OWNER AGREES TO:

1. Pay the agreed stud service fee before the bitch is serviced.

2. Notify the stud dog owner of failure to conceive, in writing, not more than 60 days following the first documented mating. Date Identified pictures showing front, rear, and side structure should be provided of Bitch and must accompany notification. If the stud is no longer available for whatever reason, for the return service, the stud owner or agent reserves the right to offer a different stud to the bitch owner. Failure to do so will result in no return service being provided.

3. The owner of the bitch warrants that this bitch has not been exposed to any stud dog prior to her arrival and that she will be protected from such exposure after her return home.

4. The owner of the Bitch agrees that puppies resulting from this breeding shall not knowingly be sold either directly, or to an agent thereof, who sells outright or consigns to a pet shop, or individual whose purpose is mass production,or wholesaling of purebred dogs.

5. Any other medical expenses that may be necessary to the bitch either for her health and safety, will also be the responsibility of the Bitch Owner. Said expenses are to be reimbursed to the Stud Owner prior to release of the bitch to her owner.

6. Any transportation costs to or from an airport or any location more than 30 miles distance from Umpqua Valley Kennels Property shall be considered an incurred cost that shall be reimbursed by owner of the bitch.

OTHER CONDITIONS

1. TWO PUPPIES BORN ALIVE OR DEAD CONSTITUTES A LITTER AND DEFINES A SUCCESFULL BREEDING.

2. FAILURE TO CONCIEVE ALLOWS FOR A RETURN SERVICE. THIS WILL BE PROVIDED AT NO CHARGE TO THIS SAME BITCH AT HER NEXT HEAT, OR WITHIN ONE YEAR, PROVIDED ALL CONDITIONS OF THIS CONTRACT HAVE BEEN MET.

2. IF THE BITCH FAILS TO CONCEIVE ON THE RETURN SERVICE, THIS CONTRACT IS TERMINATED.

The owner of the bitch hereby releases from responsibility Umpqua Valley Kennels LLC or Victor Kasser and Mary Beers or its officers from any and all liabilities and/or damages to said bitch while she is in the care of Umpqua Valley Kennels LLC for breeding.

The above agreement is made and signed this____day of____20____ by Umpqua Valley Kennels LLC and the owner of the bitch to assure the well being of the animals involved and their progeny. This contract shall be deemed a legally binding contract and is under the jurisdiction of the State of Oregon, in the County of Douglas. It is further agreed that the place of venue shall be Douglas County, Oregon.

I, We, the owner of the bitch hereby certify(s) that we have read this contract, understand it's complete contents, and agree to it fully. TO INSURE THE OWNER(S) OF THE ABOVE NAMED BITCH HAS READ AND DOES INDEED UNDERSTAND THE CONDITIONS OF THIS CONTRACT AS SET FORTH HEREIN, PLEASE SIGN AND DATE BELOW.
